CRITERIA I : NURSING KNOWLEDGE AND ADAPTABILITY VERIFICAT ION CRITERIA 1 Awareness of the departmental vision, mission, objectives and its implementation.

2 Performs the duties as perthe role and expectation of a nurse in providing comprehensive patient care. 3 Knowledge about the layout of theunit, number of patients and types of patient and their requirements.

4 Interaction, co-operation and teamspirit with other staff members. 5 Performs assigned duties as per the department’s protocol.

CRITERIA II - SOFT SKILL AND CRITICAL THINKING VERIFICAT ION CRITERIA 1 Demonstratesknowledge on AIDET (Acknowledge, Introduce, Duration, Explanation, Thank) and GST (Greet, smile, thank).

2Utilizes effective communication skills in reporting pertinent information on time. 3 - Maintains telephone etiquettes.

4 - Maintains professional decorum and communicates in English / Hindi / Bengali in the areas ofwork with colleagues. 5 Adheres to the grooming standards laid down by the department.

6 Evaluates effectivenessof nursing care on patient condition. 7 -Informs Charge Nurse / Supervisor of current activities on the unit and reportsunusual occurrence, incidents or other problems and also follows up as needed.

8- Utilizes changes of shift reportto accurately and effectively to communicate patient condition and intervention. 9Ensures all documentation is clear,concise and timely.

Utilizes appropriate clinical terminology. CRITERIA III NURSING ADMISSION ASSESSMENTVERIFICAT ION CRITERIA 1 Performs comprehensive nursing assessment on all patients withconsideration to clinical condition, pain, activities of daily living , skin allergies, integrity, valuables, safety needs, age,mental, social and physical status etc.

2 Identifies patient problems based upon data collected through nursingassessment as well as integrated information obtained through the family.

Takes measures to address any vital concernscompromising the haemodynamic stability and safety issues 3-Follows all aspects of the hospital policy in terms ofsafety , personal belongings, medico legal entity, medications etc.

4 Assesses vital signs, important informationand reports abnormalities to the consultant and team and carries out orders as per instructions.

5 Accurately andthoroughly documents nursing assessment and patient problems using appropriate clinical terminology. 6 Completesand documents ongoing assessment.

7 Assesses and communicates educational, spiritual, pain management anddietary needs to the concerned. CRITERIA IV - PLAN OF CARE VERIFICAT ION CRITERIA1 -

Formulates nursing care plan in context to care due, care given, safety first, PFE, diet and other needs in each shiftnoting patients needs, age, behavior and concerns identified through nursing assessment.

2 - Ensures that care isprovided with a comprehensive approach always ensuring safety and clinical outcomes. 3 Establishes priorities fornursing needs based on nursing assessment, problems identified and estimated length of stay.

4 - Actively participates inthe development of interdisciplinary plan of care as per policy. 5 - Ensures unmet needs are handed over to the next shiftaccurately.

6 - Acts as liaison with the treatment team and other multidisciplinary staff ensuring accurate communicationsof relevant information.

7 - Ensures evidence through accurate documentation. CRITERIA V - PROVISION OFNURSING CARE VERIFICAT ION CRITERIA 1 Practices within the general policies, philosophyand objectives of the department of nursing and the hospital with focus on safety, quality, infection control and empathy.

2 Provides nursing care in a therapeutically efficient manner demonstrating social, clinical and professional judgmentand competency.

3 Participates in the implementation of the treatment plan utilizing intervention appropriate to theneeds of the patient.

Demonstra tes knowledge and skills related to special treatment, procedures, precautions, restraints etc. 4 Provides supportive counseling of current patient concerns.

5 Assesses unit needs and maintains asafe, thera peutic environment for patients, family members and staff. 6 Assesses and meets activities of daily livingneeds of patients in dressing, bathing, grooming, toileting, eating and sleeping.

7 Identifies all vulnerable patientsand ensures that the safety first program is followed at all times. 8 Demonstrates ability to recognize different typesof incidents and responding mechanism for such issues.

CRITERIA VI- MEDICAT ION MANAGEMENTVERIFICAT ION CRITERIA 1 - Is knowledgeable about the medication process and ADR 2 Accuratelytranscribes medication and treatment orders for the indenting process.

3 Recognizes improper or inadequate ordersand reports these to supervisors and physician before proceeding 4 Checks and receives medication against theorder during pharmacy delivery.

5- Accurately and safely administers prescribed medications and treatments usingcorrect methods of administration and adheres to policy (6 R’s of medication, 2 identifiers, allergic history, double checkfor applicable medicines), admixtures , outside medication, self medication policy etc.

  • 6- Demonstra tes safe storage and control of medication. Accura tely maintains medications administra tion record. 7- Monitors the patient postadministration and reports any adverse events and accurately completes the ADR forms.
  • 8- Knowledge of Controldrugs protocol, Usage & stora ge of High alert Medications and LASA 9 Evaluates and documents patient responseto medication regimen.

    10 Implements the waste management policy 11 Is knowledgeable of action, side effectsproper dosage and contra indication of drugs being administered.

    12 Demonstra tes knowledge of and documentationrequirements for proper administra tion of medication. CRITERIA VII PROFESSIONAL SAFETY, GROWTH ANDDEVELOPMENT VERIFICAT ION CRITERIA 1 Assumes responsibility for personal andprofessional growth and development through participation at in service education programme, workshop, staff meetings,unit meeting and on the job training 2 Will adhere to all safety policies and safe work practices.

    CRITERIA VIII CUSTOMER RELATION SKILLS AND PROFESSIONAL BEHAVIOURVERIFICAT ION CRITERIA 1 Represents the hospital in a manner, which conveys a professional, courteous, caringand co operative attitude.

    2 Is knowledgeable of patient rights and responsibilities and treats all patients with dignityand respect in a consistent and caring manner.

    3 Understands the need for and maintains appropriate confidentialityat all times when interacting with patients, family, visitors and all other contacts.

    4 Exhibits excellent customer relationsskills as evidenced by supportive and constructive communication with all contacts including co-

    workers, patient, visitors,families. 5 Utilizes an open, non judgmental, non discriminatory, professional and therapeutic approach to treatmentwith all patients.

    6 Demonstrates professional behavior and interpersonal skills reflective of Apollo Hospital missionstatement and philosophy.

    7 Complies with hospital, departmental and human resources policies. 8 Promotes asafe work environment for self, patients and co-

    workers by complying with environment of care, safety, infection controland universal precaution guidelines. 9 Maintains a neat, smart professional appearance consistent with hospital dresscode.
